What information do I need to bring with me to the clinic?

Patients should carry the following information with them:

  • Insurance Card
  • National ID or Passport details
  • Referral forms from other clinician
  • Any diagnostic test results done elsewhere
  • Co-pay amount or cash for non-insured patients

How do I book an appointment?

How can I get a copy of my full medical records, for myself or to be sent to someone other than myself such as an advocate, a physician, insurance company or any other person?

To obtain copies of your medical records, please write a letter requesting a copy of them. The letter must include:

Your full name
Dates of services
Type of report needed
Your signature and stated authorization for your documents to be release to yourself or forwarded to a 3rd party

This process should take no longer than 1 week from our end.

Can I get my lab results over the phone?

Yes. Your attending clinician may call and explain your results over the phone if your results are being made available as part of a visit or a treatment episode. However, all other lab record requests can be made through the lab superintendent
